What I collect
Your contact details and year of birth, your health history and intake forms, notes from our visits, Care Plans and check-ins, and your appointment and payment records.
Why
To assess you and provide care, to book and remind you of visits, to bill you, and to meet my legal and professional duties. The legal basis is providing health care to you (GDPR Article 9(2)(h)) and your consent where I ask for it.
Who sees it
- Me, to care for you.
- Your insurer, when you ask me to send them a receipt or report.
- Other health providers caring for you, only with your consent.
- Anyone the law requires me to tell.
- Materia+, which stores my records and runs my booking and payments as my service provider. It handles your information only on my behalf. See the Materia+ Privacy Policy (https://www.materiaplus.com/privacy-policy).
How I protect it
Your health information is stored encrypted, only people who need it can see it, and access is logged.
How long I keep it
As long as the law and my professional body require.
Your choices
- See your record and get a copy.
- Ask me to correct it.
- Withdraw or limit your consent at any time.
- Ask me to delete or transfer your information, where the law allows.
